Firstly, use 'Save As' instead of 'Save' directly. Just as file system in hard disk, when you delete some files from ISO image, the allocated space by the files become empty, and can be re-used by newly added files; but the total image size keeps unchanged. 'Save As' will create a new image, which pack all unused space.

Secondly, check 'Optimize' option in 'Properties' dialoge, UltraISO will code same files in different directories only once. This feature can reduce image size dramatically when you are making All-in-one bootable CD or DVD (such as Windows 2000 3in1).

same files = same MD5 signature, just act as 'cdimage -o'.

Thank you very much! This solved my problem!
When I added a new file (quite small, less than 2M) into an existing iso file (almost 550M), UltraISO recognized its size correctly, however, Windows said its size is 0.99G! Oh my God! How can I burn it?! So I used "Save as..." command, and all the new file's attributes seemed correct!

When using "save as" instead of "save",won't it damage the cd structure which certain protections are looking for?

Yes, 'Save As' will create new CD structure for packing data to much smaller size.
