I have extracted a CD's boot image and I have made minor modifications to it (specifically to the config.sys file).  In order to effect the change, I have saved the config.sys file on my HD and then dropped the file into the Image browser.  However, UltraISO does not do anything when I try Save As... and it does not save my changes when I use the normal Save procedure.  As such, I cannot burn the modified boot image to anything.

Ideas?

Your boot image is a Disk Image, and UltraISO can not edit this kind of image until now, that's why the problem appears. Please use thirt-party program such as WinImage to modify Disk Image.
In addition, Disk Image cannot be burned to a CD directly, you should make a bootable ISO image first:
Step 1: Make a CD image from your oroginal CD, save it as disk.iso
Step 2: Save the boot image to boot.bif, rename it to boot.ima and modify something you would like to by WinImage
Step 3: Load the new boot.ima as boot image of your ISO image
Step 4: Save your ISO file and burn it as needed.
