I downloaded an ISO of Vista RC1 from Microsoft and ran an MD5 utility to verify that the file was not corrupt.  

I then wrote the ISO to DVD using UltraISO, and when I got to work used UltraISO to create a new ISO image there.

However, the MD5 of that file does not match the original.  Should I expect that round-tripping an ISO->disc->ISO will produce the same MD5, or does something (a timestamp maybe) get written to disc that's not in the ISO image?

As padding blocks may be added, please check 'Enable ISO Volume Filter' when making an ISO from disc.

This did change the MD5, but still not the same as the original.

I'm not too worried about this, unless you think that the MD5's should match.

In case it matters, the original ISO was downloaded from Microsoft, then burned using Alcohol 120%.

The 'duplicate' ISO is being created with UltraISO.

Maybe I'll try burning with UltraISO as another test.

Just tested it this way, no problem found for UltraBurn(build-in burning engine of UltraISO):
OS: Windows XP SP2
Recorder: Plextor-PX750
Disc: Verbatim 16X DVD+R
Original File: vista_5600.16384.060829-2230_x86fre_client-lr1cfre_en_dvd.iso
Original MD5: 22486e815a38feffd9667317dfeec55a
Burner: UltraBurn
Burn speed: 16X
Copy to ISO: LR1CFRE_EN_DVD.iso
New ISO MD5: 22486e815a38feffd9667317dfeec55a

Testing more:
Burner: NeroAPI
Burn speed: 8X
Copy to ISO: LR1CFRE_EN_DVD.iso
MD5:
'Enable ISO Volume Filter' ON: 22486e815a38feffd9667317dfeec55a
'Enable ISO Volume Filter' OFF: b8f02b6730341e26a6767f42fff8e9db

OK, using UltraBurn to burn the DVD and reading that DVD into an ISO with the 'Enable ISO Volume Filter' option turned on did the trick.

The MD5 on the 'duplicate' ISO matches.  Nice!
