You can make it happen too...
Install UltraISO 9.2 into "D:\Program Files\UltraISO" Run it Click Options/Configuration/Integration Check "Integrate UltraISO into shell" Click OK
Put a CD disc into your favorite CD Drive Right Click that drive in an Explorer window Choose UltraISO/Create CDDVD Image Bzzt ... UltraISO not installed(or found).
Examine ISOSHELL.DLL offset 0x99B0 The default installation path is hardcoded there. You probably already know (?)
It's an easy fix barely worthy of update; this version seems to save settings better than earlier releases. THANKS!
Other thoughts: Skinning an application is great WOW factor for the easily amused, best leftout until after the basics of whats now becoming a powerful useful application is completed. Also, ISOSHELL.DLL would be even better if it was able to put the disc label INTO the SAVE AS... dialog when using that context menu option to "Create CDDVD Image".

Just installed UltraISO into D:\UltraISO, the shell functioned properly automatically with no need to do additional setting jobs.

I'll try downloading from this website (or official mirror)...
I'll presume your typo "D:\UltraISO" means "D:\Program Files\UltraISO"
A friend gave me the installer on a flash drive.
