First, great program!  You may wish to consider "mentioning" it in the www.VMware.com user forums because .iso files are very handy for VM administrators.

I'm using UltraISO to create a smaller sized operating system install CD, for instance, I only need the files in the root of the CD and the \i386 folder .  

After Opening a CD, is there any way to directly Add selected files from multiple folders to be Saved into a new ISO image file, instead of first having to Extract the files/folders to hard drive?

Just curious if the extract to disk step can be bypassed?

Yes, latest version 7.55 supports opening a CD directly, you can add and delete files, then save to an ISO.
